The time readings are accurate and they are updated automatically inside the home screen of Earth Time Free. The live tiles you can pin to the Start screen, however, will be updated only twice in one hour, so in order to get the precise readings you will have to open the app.
A really neat feature of this utility is the ability to add any location in the world to the monitoring list and then pin it. This is done by using the built-in search function that finds those spots via the Internet, after you specify a city from the same time zone.
